Terms of Service & Telehealth Agreement

🚨 Emergency and Crisis Notice
RussellMentalHealth.com and our digital systems are not an emergency crisis intervention service. If you are experiencing a life-threatening mental health emergency, call or text 988 immediately (Suicide & Crisis Lifeline), call 911, or proceed to the nearest hospital emergency room.
1. Agreement to Terms
By accessing or using the services of Bethany R. Russell, Ph.D. & Associates, P.A. (“Practice,” “we,” “us”), including RussellMentalHealth.com, you agree to be bound by these Terms of Service, our HIPAA Notice of Privacy Practices, and our informed consent agreements.
2. Clinical Supervision & Provider Scope
Therapy services at Russell Mental Health are provided by licensed mental health counselors, qualified supervisors, and registered mental health counselor interns (RMHCI / RCSWI) practicing under the clinical supervision of Dr. Bethany R. Russell, Ph.D., LMHC-QS (Florida License MH22245). Each clinician practices strictly within the scope of Florida Statutes Chapter 491.
3. Florida Telehealth Services Consent
Telehealth services are delivered via HIPAA-compliant, encrypted videoconferencing technology to patients physically located in Florida at the time of service. You understand that:
- You must be located in the state of Florida during all telehealth sessions.
- You must have a private, secure, and confidential space with adequate internet connectivity.
- You will provide your physical location address and an emergency local contact at the start of each session.
4. Appointment Scheduling, Cancellations & No-Show Policy
Appointments may be requested online, via our voice assistant (“Eve” at 239-667-5469), or through our patient portal. We require at least 24 hours advance notice for any cancellations or rescheduling. Cancellations with less than 24 hours notice or missed appointments without notice are subject to our standard late-cancellation fee, which is not billable to insurance.
5. Payment, Insurance & Financial Responsibility
We accept select commercial and government insurance plans (Aetna, BCBS/Florida Blue, CCA, Curative, Medicare Part B, Optum, UnitedHealthcare, Step Up for Students) as well as direct-pay arrangements. You are ultimately responsible for all co-pays, co-insurance, deductibles, or non-covered services as determined by your insurance plan.
